When you call to schedule an appointment please have your prescription in front of you as you will be asked for the following information:
  • Type of exam ordered
  • Diagnosis
  • Information on any prior films of the area being scanned.
  • Your referring physician's name, address, phone and fax number
  • Patient name, address, home or business contact number.
